What to know
For all new patients, our entire staff will work to make your first visit as smooth and trouble-free as possible. To help us achieve this goal, please arrive at least 15 minutes early before your scheduled appointment. This will give you time to complete necessary patient information and medical history forms.
What to bring
There are a few important things you will need to bring with you to your appointment. These items are as follows:
- Your insurance card.
- A list of current prescriptions and/or over the counter medications you are taking, including dose and frequency. This will allow us to make sure you are on the right combination of medications.
- A list of any herbal supplements or vitamins you take on a regular basis, since any of these can affect your body and its reaction to other medications we may prescribe for you.
- Pertinent information about your medical and surgical history.
- Any recent X-rays and, if possible, please have your previous medical records sent to our office prior to your visit. This will aid the physician with your treatment.
